Teams with 13 pitchers have to occasionally "hang a pitcher out to dry" because they don't have anyone available to pitch due to having pitched on a number of recent occasions. It happens. You can't pull a pitcher at the first moment it looks like "he just doesn't have it."
I notice that you only jump up when Leland makes/doesn't make a move that doesn't turn out successfully. I notice you never seem to post "Well, I had have done X in that situation and Leland did Y, and it worked out." So basically you expect the coach of the team to never make a move that isn't successful. Not possible.
